Each school receives one (1) free coach pass allowing for free entry to the event. Additional coaches passes are $10 each. For every 10 competitors that the school has, the school will receive one additional free coach pass.

Submitting this form, the school instructor acknowledges that these fees will be calculated just before the Midwest Open and they will be responsible for payment before entry on the day of the event.

For more than 5 coach entries, please complete this form multiple times.
