By completing and submitting this form, you declare that you are either the competitor, or you are an agent with permission to act on behalf of the competitor. You understand that submission of this registration form demonstrates that you have read and agree to the terms of this liability release. You do hereby release and discharge all rights and claims for any damage that you or the client you a representing may receive while participating in this martial arts activity. You hereby release Joshua Sharp, Sharp's Taekwondo, Decatur Conference Center and Hotel, and any representatives or officials involved in this tournament. You, or the client you are representing, do hereby declare that you are competing at your own risk and that you are physically and mentally fit for these events. You, or the client you are representing, have consulted with a physician and able to participate in these events. You assume all risks, including injury or death. You understand that sparring is a contact sport and by participating, injuries may be incurred. By submission of this registration form, an agreement is submitted that the competitor listed on this form waives any and all claims against the aforementioned. This agreement also includes that Joshua Sharp, Sharp's Taekwondo and Decatur Conference Center and Hotel or any representative from this tournament are not liable in any way for any harm incurred to or on my person in the course of travel to and from this event.
