2: PRINT & SIGN
You can fill out the registration form on the computer and print it out. You can also print it and then complete it by hand. Be sure to complete the form by signing at the bottom of the corresponding page. Please read the packet carefully and fill out all fields.
3: RETURN & PAY
Submit the 2025 Midwest Open registration packet along with payment in full to your school instructor or directly to Master Sharp.
